Supporting documents accompanying the customer's application for subscription of the Service shall be required to be submitted to tmnet together with the Application Form. The required supporting documents are as follows: For Malaysian: Individual - a copy of the individual's Identity card (both side) and phone bill. Sole proprietor or partnership - a copy of either the Customer's business license, form D, B, or A; Company - a copy of Form 9, Form 49, Form 13. Installation upon successful, customer will be charged RM125, which will appear in the first monthly bills. The 1st bill will include installation and service activation fee of RM125, monthly subscription and pro-rate of monthly subscription fee.
